BACKGROUND
The City entered into Subrecipient Agreement 24-006 for fiscal years 2024-2027 with Clackamas
County Social Services Division on September 5, 2023. The first amendment (Amendment 1),
Resolution 24-21 was signed on May 7, 2024, providing $21,200 in funding for fiscal year
2023/2024. A second amendment (Amendment 2), Resolution 24-31 was signed on June 18, 2024
increasing the grant allotment to $161,920. The third amendment (Amendment 3), Resolution
25-21 approved on May 6, 2025, increased the grant by $25,856, for a total amount of
$187,776 available for the 24/25 fiscal year. The fourth amendment (Amendment 4), Resolution

25-38 approved on September 2, 2025 increased the grant by$151,420 available for the 25/26
fiscal year. Pursuant to that agreement, some of the human services the City offers through the
Adult Community Center (ACC)—such as Meals on Wheels, respite,transportation and information
assistance—are paid for by the City and then reimbursed at a set rate by Clackamas County with
Federal and State funds. Clackamas County received additional funding for the purpose of providing
human services.
DISCUSSION
The purpose of the fifth amendment (Amendment 5) is to increase the grant funds for the fiscal
year of 2025/2026 by$7,440 for home delivered meals. Amendment 5 also increases the grant
funds for fiscal year 2026/2027 by$149,085 by Clackamas County for reimbursable services
provided by the ACC.
FISCAL IMPACT
The fifth amendment is our initial allotment for the fiscal year 26/27 and a slight addition of
funds for the fiscal year 25/26. The ACC often provides more services than the County allots for
reimbursement. The reimbursement is not dollar for dollar spent, but it helps cover the costs
ACC incurs when providing services.
RECOMMENATION
Adopt Resolution 26-22.

A RESOLUTION OF THE CITY COUNCIL OF THE CITY OF LAKE OSWEGO AUTHORIZING THE MAYOR TO
EXECUTE AMENDMENT NO. 5 TO THE SUBRECIPIENT AGREEMENT 24-006 BETWEEN THE CITY AND
CLACKAMAS COUNTY HUMAN SERVICES DEPARTMENT'S SOCIAL SERVICES DIVISION FOR FISCAL
YEARS 2023-2027.
WHEREAS,there is a need to provide social services to the citizens in Lake Oswego; and
WHEREAS, Clackamas County has the funding available to provide social services to citizens in Lake
Oswego through the Older Americans Act; and
WHEREAS,the City and Clackamas County have previously entered into intergovernmental agreements,
Agreement 24-006 signed on September 5, 2023, for fiscal years 2023-2027 Amendment 1 (Resolution
24-21) signed on May 7, 2024, for fiscal year 2023-2024 and Amendment 2 (Resolution 24-31) signed on
June 18, 2024, for fiscal year 2024-2025 whereby the City has provided social services; and Amendment
3 (Resolution 25-21) signed on May 6, 2025; and Amendment 4 (Resolution 25-38) signed on September
2, 2025;
NOW,THEREFORE, BE IT RESOLVED by the City Council of the City of Lake Oswego that:
Section 1. The Mayor is authorized to execute on behalf of the City of Lake Oswego Amendment
No. 5 to the Subrecipient Agreement 24-006 with Clackamas County Health, Housing and Human
Services, Social Services Division for fiscal years 2023-2027, in the form attached as
Exhibit A.
Section 2. Effective Date. This Resolution shall take effect upon passage.
Considered and enacted at the regular meeting of the City Council of the City of Lake Oswego on the 19th
day of May, 2026.

Justification for Amendment:
This Amendment#5 is entered into by and between the City of Lake Oswego ("SUBRECIPIENT") and Clackamas
County("COUNTY")and shall become part of that federal subrecipient agreement("Agreement")entered into by and
between the parties on 9/28/23. Amendment#5 revises the FY26 budget and adds the FY27 budget, resulting in an
increase to the budget by an additional $156,525, for a total Agreement amount of$649,091.
This Amendment#5 is effective upon signature. Except as amended hereby, all other terms and conditions of the
Agreement remain in full force and effect. COUNTY has identified the changes with "bold/italic"font for easy
reference.
AMEND Section 4, Grant Funds:
4. Grant Funds. COUNTY's funding for this Agreement is a combination of Federal, State and Local
dollars as specified below by title and Assistance Listing Number("ALN") number as appropriate.
The maximum, not to exceed, grant amount COUNTY will pay for is $492,566. Payments will be
made on a reimbursement basis in accordance with the rates set forth in Exhibit B, and the award
is conditional upon compliance with the terms herein and disbursements will be made in
accordance with the schedule and requirements contained in Exhibit D: Reimbursement Request.
Failure to comply with the terms of this Agreement may result in withholding of payment.
4.1.Grant Funds: COUNTY's funding of$374,610 in grant funds for this Agreement is the Older
Americans Act(ALN: 93.043, 93.044, 93.045, 93.052, 93.053) issued to COUNTY by the
State of Oregon, Department of Human Services ("ODHS"), Adults and People with
Disabilities ("APD"), Community Services & Solutions Unit("CSSU").
4.2.Grant Funds: COUNTY's funding of$27,200 in grant funds for this Agreement is the
Medical Assistance Program issued to COUNTY by ODHS, APD.
4.3.Grant Funds: COUNTY's funding of$33,000 for transportation services outlined in this
agreement are from Statewide Transportation Funds ("STF")for Elderly and Disabled and
Statewide Transportation Improvement Funds("STIF")for transportation services issued to
